Dynamic stamp; Reorder input text field or show it in one window

I have made a dynamic stamp with different text field. When I insert the stamp in a document the program opens for every text field his own fill in window.

I want one window with all the input text field under each other

How can I do this?

How can I reorder the input text field order when it is not possible all the input text field to show in one window.
